A pilot initiative launched by the Rockford Fire Department in collaboration with SwedishAmerican Hospital has shown promising results in reducing the number of emergency room visits by "superusers"—patients who frequently rely on ambulance services. While the program has been successful, the challenge of securing long-term funding for its expansion remains unresolved.
The program targeted eight out of approximately 380 patients at SwedishAmerican who visited the emergency department more than 10 times a year, often making dozens of trips. These individuals received home visits from nurses and firefighters during the first half of the year. According to Dr. Kathleen Kelly, chief clinical integration officer at SwedishAmerican, these visits led to a significant decrease in ER visits and ambulance use.
Known as the Mobile Integrated Healthcare pilot program, it focused on a small portion of the population that frequently uses ambulance services in Rockford. The primary goal was to cut down on unnecessary ER visits and ambulance rides while improving the overall health of the participants.
"After our team conducted proactive home visits to identify the triggers behind these frequent emergency department visits, we saw a substantial reduction in both ER admissions and ambulance transfers," Kelly explained.
Officials believe that expanding the program—potentially with support from all three major healthcare systems in the region—could be highly effective. This aligns with findings from similar mobile healthcare programs implemented across the country, which have also demonstrated positive outcomes.
During the first six months of 2014, the eight patients involved in the pilot program visited the emergency room a total of 65 times. By the same period in 2015, this number dropped to 30—a 54% decrease. Additionally, they used ambulances 30 times in the first half of 2015, compared to 48 times in the same timeframe the previous year, representing a 38% reduction.
All eight participants suffer from complex medical conditions that require ongoing medication. However, according to Kelly, many of their issues could be better addressed through regular visits to a primary care physician, leading to improved health outcomes and more efficient use of healthcare resources.
Many of these patients live in poverty or have limited incomes, which can make accessing healthcare and transportation difficult. Simply giving them a direct line to contact nurses helped reduce the number of 911 calls, as they were able to get timely assistance without needing emergency services.
This program highlights the potential of community-based healthcare solutions in addressing systemic challenges within the emergency care system. As more data becomes available, there is hope that such initiatives can be scaled up to benefit a broader population.
